Systemische reacties op in de mond toegepaste metaallegeringen

In de mond toegepaste metaallegeringen kunnen ongewenste lichamelijke effecten veroorzaken. Daarbij wordt onderscheid gemaakt tussen lokale en systemische reacties en toxische en immuunreacties. Een gepresenteerde casus illustreert de problematiek. In deze casus is waarschijnlijk het aanbrengen van orthodontische apparatuur de aanleiding geweest voor een exacerbatie van nikkelallergie. Het contact met nikkel resulteerde in handeczeem. De patiënt was ook via enkele kronen, een frameprothese en via de voeding met nikkel in contact gekomen, waardoor het verwijderen van de orthodontische apparatuur niet in volledige genezing resulteerde. De nikkel bevattende kronen moesten daarom ook worden verwijderd en de voeding moest voortaan in nikkelvrije pannen worden bereid.

Systemic reactions to orally applied metal alloys
Orally applied metal alloys can cause undesirable physical effects. A distinction needs to be made in this respect between local and systemic reactions and toxic and immunological reactions. A case is presented which illustrates this problem. In this case, the application of orthodontic appliances was probably the trigger for an exacerbation of nickel allergy. The oral exposure to nickel resulted in hand eczema. The patient was also exposed to nickel by single-unit fixed dental prostheses, a removable dental prosthesis, and food, as a result of which removal of the orthodontic appliances did not result in complete healing. Therefore, the single-unit fixed dental prostheses also had to be removed and food had to be prepared henceforward in nickel free pans.
